29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  On or about March 8, 2021, west side of site, employees were using a 8' x 4' make shift trash box attached to the the forks of a JCB rough terrain forklift (#10382707) as  a  scaffold platform.  The employees were being lifted to the roof top approximately 42 feet high.  The platform was not equipped with a guardrail system and the employees were not using personal fall arrest systems.
