1926.651(k)(1) An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: On or about March 8, 2021, east wall of trench, employee(s) installing drainage culverts were exposed to a crush-by hazard. The competent person on site did not removed the employee(s) from the hazardous area until the necessary precautions were taken to ensure their safety.
