29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Victor Segura Gonzalez located at 2900 Martin Road, Dublin, OH 43017:  On and preceding March 22, 2021, the employer did not ensure that employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects utilized head protection. Roofers were removing  shingles on a church roof that varied from 12-35 feet with the employees working at different levels.  The employees were not utilizing proper head protection which exposed the them to struck by hazards to the head.

29 CFR  1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):   a) Victor Segura Gonzalez located at 2900 Martin Road, Dublin, OH 43017:  On and preceding March 22, 2021, the employer did not ensure that an enclosed chute was used where roofing shingles and other debris were dropped approximately 35 feet into a dumpster lying outside the exterior walls of the building.  The following exposed employees to struck by hazards and increased the risk of falls off of the edge of the building where they were throwing materials from.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) Victor Segura Gonzalez located at 2900 Martin Road, Dublin, OH 43017:  On and preceding March 22, 2021, the employer did not ensure that employees engaged in construction activities 6 feet or more above lower level were protected from falls. Roofers were removing and disposing of shingles and other roofing materials at heights up to 35 feet on a church roof without fall protection. The following condition exposed them to falls up to 35 feet.
