29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Roof - On or about March 18, 2021 employer allowed employees to work from  flat roof installing flashing  along open edge  of roof without fall protection or a safety monitor exposing employee to fall hazard of 14'7".  (b) Roof - On or about March 19, 2021 the employer allowed employee to use a torch to seal ductwork on roof without fall protection or a safety monitor exposing employee to a fall hazard of 25 feet.

29 CFR  1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:   (a) Roof -  On or about March 19, 2021 the employer allowed employee to use a torch to seal ductwork on roof without fall protect or a safety monitor above unguarded rebar protruding from the foundation level of the building and at top of stairs below roof edge exposing employee to a impalement hazard.
