29 CFR  1910.146(d)(5)(i) to (vi):  Test conditions in the permit space to determine if acceptable entry conditions exist before entry is authorized to begin, except that, if isolation of the space is infeasible because the space is large or is part of a continuous system (such as a sewer), pre-entry testing shall be performed to the extent feasible before entry is authorized and, if entry is authorized, entry conditions shall be continuously monitored in the areas where authorized entrants are working;  On or about March 22, 2021, in the shop in Colorado City, Texas, employee enter a permit required confined space without the conditions of the ISO tank being tested before entry, exposing employee to inhalation hazards.

29 CFR  1910.146(d)(6): Provide at least one attendant outside the permit space into which entry is authorized for the duration of entry operations;   On or about March 22, 2021, center bay of shop in Colorado City, Texas, employer did not ensure a designated attendant was posted at the entrance of the tank in case of an emergency as an employee was pressure washing the inside of the tank.

29 CFR  1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:  On or about March 22, 2021, in the shop in Colorado City, Texas, an employee entered a permit required confined space prior to entry permit being performed, exposing the employee to respiratory  inhalation hazards.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   On or about March 22, 2021, employees were exposed to inhalation and chemical hazards as the employer  failed to ensure employees working in and around hazardous chemicals were trained.
