Employees were exposed to fire and explosion hazards associated with exposure to combustible dust when production processes produced combustible dust.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: a. On or about March 23, 2021, in the facility, employees were not provided with training on the measures to be taken to protect themselves from combustible dust hazards. Employees were exposed to fire and explosion hazards when not provided with training, which included, but was not limited to, appropriate work practices to be used for the prevention of combustible dust cloud generation and dust accumulation throughout the workplace.
