29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) On or about March 24, 2021, at the above addressed jobsite, an employee performing siding operations from a tubular welded frame scaffold was working from a work deck that was not fully planked, thereby exposing the employee to a six (6) feet (6) inches fall hazard.

29 CFR  1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about March 24, 2021, at the above addressed jobsite, an employee performing siding operations from the roof of a canopy was not protected from fall hazards, thereby exposing the employee to fall hazards of twelve (12) feet five (5) inches.
