29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  Wide line coater room, Waldron top-side coater - on or about March 17, 2021, the control of hazardous energy lockout/tagout authorized employee training was not provided to coater operators and attendants to understand the purpose and use of the energy control procedure, and the methods and means necessary for energy isolation and control during color changeovers on energized equipment.  Employees had their hands exposed to caught-in hazard of the in-going nip point between the applicator and pickup rollers likely to result in serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   (1) prepare for shut down by recognizing energy hazards and its controls;  (2) turn off and shut down the machines; (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; (4) affix lockout or tagout devices to each energy isolating device;  (5) render safe all potentially hazardous stored or residual energy; and,  (6) verify that isolation and de-energization of the machine or equipment have been accomplished.   Wide line coater room, Waldron top-side coater - on or about March 17, 2021, the employer did not ensure that energized equipment had its hazardous energy source(s) isolated prior to the coater operators and attendants beginning their color change tasks which included wiping rotating rollers by hand. Employees had their hands exposed to caught-in hazard of the in-going nip point between the applicator and pickup rollers likely to result in serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Recoil lines: on or about July 14, 2021, employees had their legs and hands exposed to in-going pinch points where the material was fed onto rotating parts without adequate safeguards used to prevent access. Employees stood at the control station to adjust speed or walked in the adjacent aisle within reach to the unguarded elements of the line running normal production.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
