On or about March 23, 2021, and at times prior, employees were working inside of an excavation between an unprotected excavation wall and a rebar wall to tie rebar and lay concrete without conducting an inspection of the excavation and adjacent areas prior. As a result, employees were exposed to cave-ins and associated caught-in and struck-by hazards.
