29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent; a) Turnpike Station 3601 Pickett Road Fairfax, VA: On or about May 18, 2021, the employer failed to enter the recordable COVID-19 case on the OSHA 300 Log for the calendar year of 2021. b) Turnpike Station 3601 Pickett Road Fairfax, VA: On or about May 21, 2021, the employer failed to enter the recordable COVID-19 case on the OSHA 300 Log for the calendar year of 2021. c) Turnpike Station 3601 Pickett Road Fairfax, VA: On or about May 24, 2021, the employer failed to enter the recordable COVID-19 case on the OSHA 300 Log for the calendar year of 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
