Medina, OH, 44256. Employees were performing roofing work on a garage and were exposed to a fall of approximately 8 feet without adequate fall protection. Employees were performing roofing work on the residence and were exposed to a fall of approximately 12 feet without adequate fall protection.

Medina, OH 44256., was 3 feet above the landing surface. Employees were performing roofing work on the residence and were exposed to a fall hazard of approximately 12 feet.
