29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   On or about March 24, 2021, and at times prior thereto employee(s) engaged in residential construction activities in aerial lifts were not protected by fall protection systems and exposed to the following unprotected fall hazards:  1) Employee was witnessed working in an aerial lift approximately 20 feet in the air at Building 12. Employee was wearing a body harness with no lanyard seen or attached to the aerial lift basket.   2) Employee was witnessed standing inside of an aerial lift work basket with no body harness worn, level with the third floor of apartment Building 1, approximately 30 feet in the air.
