29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Worksite located at 2 Dakota Circle, Lake Winnebago, Missouri- Employee  was performing a walk-around of the residence and was exposed to multiple overhead hazards from employees and equipment and was not wearing any means of head protection.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Worksite located at 2 Dakota Circle, Lake Winnebago, Missouri- Employees were exposed to a fall hazard while working on a steep roof and were not protected from falling by the use of any means of fall protection.

29 CFR  1926.1052(c)(1)(ii):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:   Worksite located at 2 Dakota Circle, Lake Winnebago, Missouri- Employee was exposed to a fall hazard in that the stairway going up to the 2nd level of the home was not provided with stairrail system.
