29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about March 23, 2021, employees were exposed to a struck by hazard while performing siding activities on a residential home located at 9170 Winfield, North Ridgeview, Ohio 44039. Employees were working on upper areas with tools and materials that could have fallen on lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.
