In the alternative...  29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage:  (a)  On or about March 23, 2021, the employer did not ensure the Computer Control Sheet Cutter "Chopper #5" (Model # CQ 360 and Serial #400025) was properly shut down utilizing a hazardous energy control program procedure (Lockout/Tagout), which exposed employees to caught-in/crushing hazards and amputation injury.
