29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Northcoast Handyman Service LLC, on or about March 24th, 2021, at the location of 3404 Milan Rd., Sandusky, OH, 44870: Employees were performing roofing work while exposed to fall hazards of approximately 14 feet to the ground below without the benefit of fall prevention training to enable them to identify and mitigate these hazards.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Northcoast Handyman Service LLC, on or about March 24th, 2021, at the location of 3404 Milan Rd., Sandusky, OH, 44870: Employees were performing roofing work while exposed to fall hazards of approximately 14 feet to the ground below without the benefit of fall protection.

29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  (a) Northcoast Handyman Service LLC, on or about March 24th, 2021, at the location of 3404 Milan Rd., Sandusky, OH, 44870: Employees were performing roofing work while exposed to fall hazards of approximately 14 feet to the ground below without the benefit of accident prevention programs to identify and mitigate these hazards.   Northcoast Handyman Service, LLC was previously cited for a violation of this occupational safety and health standard, 1926.20(b)(1), which was contained in OSHA inspection number 1485522, citation number 1, item number 1, and was affirmed as a final order on 9-29-2020, with respect to a workplace  located at 115 W Water St., Sandusky, OH, 44870.
