29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 23, 2021,  employees were using pneumatic roofing nailers to install shingles on a residential roof under construction without the use of eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about March 23, 2021, three employees were installing shingles on a residential roof under construction at heights above 6 feet without the use of fall protection, exposing employees to a fall hazard.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about March 23, 2021, three employees were installing shingles on a residential roof under construction at heights above 6 feet without receiving training from the employer, exposing employees to a fall hazard.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    On or about March 23, 2021, employees were accessing the roof of residential home by means of a ladder that did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard.
