29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): a) Valley View Manor Nursing Home located at 40 Park St, Norwich, NY: From 1/7/21 through 4/1/21: The employer permitted the use of Sekura N95 respirator after a Certified Nurse's Assistant (CNA) failed a fit-test and was neither retested on nor provided with other respirator models or types. The employer required the CNA to wear a N95 respirator to protect against SARS-CoV-2 virus when providing care to suspected or confirmed COVID-19 residents. b) Valley View Manor Nursing Home located at 40 Park St, Norwich, NY: From 1/7/21 through 1/8/21: The employer permitted the use of Sekura N95 respirator after a Nurse Manager failed a fit-test and was neither retested on nor provided with other respirator models or types. The employer required the Nurse Manager to wear a N95 respirator to protect against SARS-CoV-2 virus when providing care to suspected or confirmed COVID-19 residents. c) Valley View Manor Nursing Home located at 40 Park St, Norwich, NY: From 1/15/21 through 2/28/21: The employer permitted the use of Sekura N95 respirator after a Medical Specialist failed a fit-test and was neither retested on nor provided with other respirator models or types. The employer required the Medicaid Specialist to wear a N95 respirator to protect against SARS-CoV-2 virus when interviewing and triaging suspected or confirmed COVID-19 residents. d) Valley View Manor Nursing Home located at 40 Park St, Norwich, NY: From 1/19/21 through 3/31/21: The employer permitted the use of Sekura N95 respirator after a Licensed Practical Nurse (LPN) failed a fit-test and was neither retested on nor provided with other respirator models or types. The employer required the LPN to wear a N95 respirator to protect against SARS-CoV-2 virus when providing care to suspected or confirmed COVID-19 residents. Abatement certification must be submitted for this item.
