On or about March 22, 2021, at the location of 3155 Chillicothe Lancaster Road South West, Lancaster, Ohio:  the employer failed to ensure that an employee was protected and not exposed to struck-by hazardous conditions when an employee attempted to clear the concrete blockage of the concrete pump truck, Make: Putzmeister, Model: BSF 38Z.16H, Model Year:  2021, Serial No. 210606086.  The employee was struck-by the sudden release of air and concrete as a result.
