29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   Location - 4 Republic Road, Billerica, MA.  On or about March 23, 2021, two employees were working from a  scaffolding without fall protection.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   Location - 4 Republic Road, Billerica, MA.  On or about March 23, 2021, two employees were working/walking on a roof at a height greater than twelve feet above lower level and were not protected from falling.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Location - 4 Republic Road, Billerica, MA.  On or about March 23, 2021, two employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing.
