29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                            a}  Exterior roof: Employees were exposed to eye/face injuries while using pneumatic nail guns and were not protected with any type of eye/face protection, on or about 3/22/21.

29 CFR 1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and/or did not provide for the firefighting equipment as specified in this subpart:  a) Hisham Gadalla, does not ensure that employees have available firefighting equipment at the jobsite. The General Contractor did not provide any firefighting equipment at the jobsite. Combustible products such as wood were used. Gasoline fueled generators was used on site next to the building entrance to power electric equipment such as, nail guns, extension cords and compressors to perform roofing activities, on or about 3/22/21.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Building Exterior - Roof: Employees were exposed to a 30 foot fall hazard while working on a  roof of a residential home without the use of personal fall arrest system or other means of fall protection in place, on or about 3/22/21.
