29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At the jobsite,  employees were exposed to fall hazards of approximately 20-feet and greater when they used an aluminum extension ladder at the front of the house that was not set three feet above the roof landing.   b) At the jobsite,  employees were exposed to fall hazards of approximately 13-feet and greater when they used an aluminum extension ladder at the front right side of the house that was not set three feet above the roof landing.   c) At the jobsite,  employees were exposed to fall hazards of approximately 21-feet 3-inches and greater when they used an aluminum extension ladder at the rear right side of the house that was not set three feet above the roof landing.   d) At the jobsite,  employees were exposed to fall hazards of approximately 12-feet 3-inches and greater when they used an aluminum extension ladder at the rear of the house that was not set three feet above the roof landing.
