29 CFR  1910.29(b)(2)(i):(i) Midrails are installed at a height midway between the top edge of the guardrail system and the walking-working surface;  On or about March 24, 2021, the employer failed to install a midrail at a height between the toprail and the walking-working surface (floor) around the bed frame paint dip tank.  The paint dip tank was approximately 83 inches deep or greater.

On or about March 24, 2021, the employer failed to develop, document, and/or utilize procedures for the control of potentially hazardous energy when engaged in service and maintenance activities on machinery and equipment at the facility.  The employer engages in the service and maintenance activities on machinery and equipment including, but not limited too; mechanical power presses, riveter machines, gluing table machines, etc.

29 CFR  1910.147(c)(7)(i):The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  On or about March 24, 2021, the employer failed to provide training to ensure affected and authorized employees understood the purpose and function of the energy control program and employees had acquired the knowledge and skills required for the safe application, usage, and removal of energy controls.  The employer engages in the service and maintenance activities on machinery and equipment including, but not limited too; mechanical power presses, riveter machines, gluing machines, etc.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   a.  On or about March 24, 2021, the employer did not ensure one or methods of machine guarding was provided to protect the operator from hazards created by the point of operation of the A2 "shoulder" riveter machine.  b.  On or about March 24, 2021, the employer did not ensure one or methods of machine guarding was provided to protect the operator from hazards created by the point of operation of the C1 "leg" riveter machine.

On or about March 24, 2021, the employer did not ensure point of operation guarding was properly applied to the point of operation of the P2 Minister 45ton, part-revolution mechanical power press.    b.  On or about March 24, 2021, the employer did not ensure point of operation guarding was properly applied to the point of operation of the P3 Niagara, part-revolution mechanical power press.     c.  On or about March 24, 2021, the employer did not ensure point of operation guarding was properly applied to the point of operation of the P4 L&J, part-revolution mechanical power press.

29 CFR  1910.219(f)(3):Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  On or about March 24, 2021, the employer did not ensure the chain and sprocket wheels used for the power transmission between the No. 2 West Glue Table electrical motor and conveyer head pulley were enclosed (guarded).  The chain and sprocket wheels were less than seven feet above the shop floor.
