29 CFR 1910.305(g)(1)(iv)(E): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be concealed behind building walls, ceilings, and/or floors: a) North Pacific Fuel Dock - On or about March 29, 2021, employees were exposed to electrical and fire hazards while tally counting in the portable count house. Power was supplied by a 110 volt extension cord, but the extension cord used was observed running along the ground and concealed underneath the count house building and debris.

29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets were not visually inspected before use on any shift for external defects and for evidence of possible internal damage: a) North Pacific Fuel Dock - On or about March 29, 2021, employees tally counted in the count house with power supplied with the use of a flexible extension cord. The flexible extension cords were not visually inspected prior to use for external defects to include damage to the outer jacket and evidence of possible internal damage prior to use, exposing employees to electrical and fire hazards.
