Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  On March 25, 2021 at 2045 West 20th Avenue in Oshkosh, Wisconsin, employees were exposed to fall hazards when engaged in roofing activities without utilizing required fall protection equipment.  Employees were exposed to unprotected roof edges thirty feet above surface level on the east, west and north sides and seventeen feet above a lower roof surface at the northeast corner.

Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.   a)  On March 25, 2021 at 2045 West 20th Avenue in Oshkosh, Wisconsin, no warning lines were installed between the roof work area warning lines and the top of an access/egress ladder that was positioned on a lower roof surface.  Employees were using the ladder to access/egress the roof edge seventeen feet above the lower roof surface.  b)  On March 25, 2021 at 2045 West 20th Avenue in Oshkosh, Wisconsin, no warning lines were installed on a lower roof surface for the access/egress path between the top of a ladder positioned at surface level and the base of a ladder positioned on the lower roof surface.  Employees were walking on the lower roof surface with unprotected edges seventeen feet above surface level.
