Employees were operating a Volvo 2200 model 7DZXL06-1063 Fork Truck to move and stack tree trunks in storage areas in the main yard and the employer did not ensure that the operators were trained to operate the Fork Truck safely.

Employees were operating a Volvo 2200 model 7DZXL06-1063 Fork Truck to move and stack tree trunks in storage areas in the main yard and the employer did not ensure that the operators were trained to look in the direction of travel during operation.

Employees were operating a Volvo 2200 model 7DZXL06-1063 Fork Truck to move and stack tree trunks in storage areas in the main yard and the employer did not ensure that the vehicle was taken out of operation when it was found to have a defective back up alarm.
