29 CFR 1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): Employees were exposed to electrical hazards. Two employees locked out the Nothum Breading Machine in Further Processing Line 5. Their lockout tagout devices did not identify the employee applying the device. One employee's tag used with the lockout tagout device had multiple names. The other employee's tag used with their lockout tagout device was blank with no name or identification.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: An employee was exposed to amputation hazards. Two guards were absent on the Nothum Breader Machine # 2 on Fully Cooked Line 5. a) An employee was exposed to a missing guard on the Nothum Breader Machine #2.There was no infeed guard preventing access to the incline chain. b) An employee was exposed to a missing guard on the Nothum Breader Machine # 2. The side guard above the sifter belt that blocked the incline chain was absent. On or about March 22, 2021, an employee received a partial amputation of their finger when unclogging the breading on the Nothum Breader Machine # 2 on the Fully Cooked Line 5.
