Lafayette, NJ Employees were exposed to amputation hazards as the "Dust silo/dust weigh hopper" did not have machine specific Lock Out / Tag Out procedures that were effectively communicated and understood by the employees working on the machine resulting in a workplace injury on or about 03/26/2021.
