29 CFR 1910.119(f)(4):The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On or around March 29, 2021 in the AN2 Unit, the employer did not ensure safe work practices were implemented while contract employees were repairing a piping flange leak on the 30 R1-3 Superheat steam coil line, in that: (a) The employer did not require all persons entering the work area to utilize stipulated PPE for line break to mitigate hazards. (b) The employer did not have owning area representative (e.g., Operator) present for the initial break-in to the system to eliminate possibility for exposure or release. 29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about March 29, 2021 on the coil deck of Reactor 3 in the AN2 Unit, workers were allowed to enter into the immediate area near an opened steam line and did not use PPE needed to protect them in the event they made contact with steam and or hot condensate. (a) Contract maintenance workers in the work zone around the flange in the superheat steam coil piping system.
