On or about March 30, 2021, inside USMS office 202A at the upper level of the Federal Operations building, San Juan, PR, two Relocatable Power Taps were serially-connected and not directly connected to a permanently installed branch circuit receptacle. c. On or about March 30, 2021, in USMS Reception Area at the upper level of the Federal Operations building, San Juan, PR, two Relocatable Power Taps were serially-connected and not directly connected to a permanently installed branch circuit receptacle. d. On or about March 30, 2021, inside USMS Asset Forfeiture Unit Room at the upper level of the Federal Operations building, San Juan, PR, six Relocatable Power Taps were serially-connected and not directly connected to a permanently installed branch circuit receptacle. e. On or about March 30, 2021, at the USMS lower level area in the Federal Operations building, San Juan, PR, the 10-pound, ABC dry chemical portable fire extinguisher, mounted on the column between the refrigerator and the paper shredding machine, was not subjected to an annual maintenance check (maintenance check record tag was expired).
