29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Faustson Tool Corp. at 6803 Joyce St, Arvada, CO 80007: On or before 3/31/21 the employer did not protect employees from injury in that the Bridgeport mill [Serial: J320345] was not equipped with a chip guard. A folded piece of cardboard was used by some but not all employees as a chip guard. This condition exposed employees to flying chips and sparks as well as laceration and amputation hazards. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

The employer utilized an outdated program lacking current requirements under the Globally Harmonized Standard (GHS). This condition exposed employees to hazardous chemicals including but not limited to: -acetone -isopropyl alcohol -denatured alcohol -WD-40 Abatement Note: Abatement certification is required for this item. 29 CFR 1910.303(g)(1)(i):Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less: (a) Faustson Tool Corp. at 6803 Joyce St, Arvada, CO 80007: On or before 3/31/21 the employer did not ensure that electrical service panels operating at 600 volts or less had at least 30 inches of clearance. Two areas in the shop where electric service panels were located were blocked and or had items stored within 30 inches of the panel. This condition exposed employees to electric shock and fire hazards. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
