At a temporary worksite, located at, or near, 3525 Imeson Road in Jacksonville, Florida: On or about April 1, 2021, the employer exposed employees to a 27-foot fall hazard, in that a powered industrial truck (forklift) operator dismounted a JCB brand, model 507-42, telescopic handler (rough terrain forklift), with a personnel work platform that was elevated to approximately 27-foot and contained two employees.

At a temporary worksite, located at, or near, 3525 Imeson Road in Jacksonville, Florida: On or about April 1, 2021, and at times prior, the employer exposed employees to a fall hazard, in that a personnel work platform was used by employees installing sheet metal siding from a GEHL brand, model 663 Dynalift Telescoping Boom Forklift Truck which was not design by the manufacturer for such use.
