29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Fabrication Shop - On or about March 31, 2021, employees used a 120V AC Tri-Tap Connector (model 6yF68) with an SJTW 14/3 extension cord to energize lighting, battery charging station, and portable hand tools.  The end plug was series chained to another extension cord and was not directly plugged into a receptacle in accordance with manufacturer instruction and the UL labeling requirements, exposing employees to electrical hazards.

29 CFR  1910.303(g)(1)(i)(C): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of work space was not clear and did not extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of 29 CFR 1910.303:   a) Electrical Room - On or about March 31, 2021, the working space in front of a three-phase 120 volt panel labeled Panel CR-1, had a garbage can and five (5) gallon bucket in front of it, exposing employees to electrical and fire hazards.  b) Electrical Room - On or about March 31, 2021, the working space in front of a 480 volt distribution panelboard labeled T1-2-3, had a desk, computer chair, letter storage, and energy control locks and tags in front of it, exposing employees to electrical and fire hazards.  c) Electrical Room - On or about March 31, 2021, the working space in front of 240 volt panel labeled WP-2 had a box and garbage can in front of it, exposing employees to electrical and fire hazards.  d) Fabrication Shop - On or about March 31, 2021, the working space in front of a breaker panel N had a Smartwasher parts washer in front of it, exposing employees to electrical and fire hazards.

29 CFR  1910.305(g)(1)(iv):Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a):  a) Powerhouse - On or about March 31, 2021, a flexible extension cord was placed through a hole in the wall and used as a means of fixed wiring to provide power to computer equipment, exposing employees to electrical and fire hazards.  b) Powerhouse - On or about March 31, 2021, a flexible cord was  used as a substitute for fixed wiring to energize a 120V SIMPLX packaged pump set (model: SPS-20).  The flexible cord was painted with white paint and placed behind metal brackets attached to the wall and through holes in the ceiling area, exposing employees to electrical and fire hazards.  c) Fabrication Shop - On or about March 31, 2021, a flexible cord of a LumaPro 50 ft. tri-tap cord reel (model 6YF68) was used as a substitute for fixed wiring to energize lighting, battery charging station, and powered hand tools.  The flexible cord was placed through and concealed in a wall and door opening.  These conditions exposed employees to electrical and fire hazards.  d) Fabrication Shop - On or about March 31, 2021, a flexible red extension cord was used as a substitute for fixed wiring to energize a refrigerator.  The extension cord was attached to the walls with metal bracing and nails, exposing employees to electrical and fire hazards.

29 CFR  1917.95(c):The employer did not provide well maintained emergency bathing, eye washing or other facilities where employees were exposed to hazardous substances.  a) Top Pick Shed - On or about March 31, 2021,  employees were provided a Honeywell PortaStream II Emergency Eyewash Station (SN: 616988) to flush eyes in the event of an emergency.  The eyewash saline solution was not maintained in accordance with manufacturer requirements, exposing employees to biological hazards.  b) Powerhouse - On or about March 31, 2021,  employees were provided a Sperian Fendall Flash Flood Catridge emergency eyewash station (SN: 414171)  to flush eyes in the event of an emergency.  The eyewash saline solution was not maintained in accordance with manufacturer requirements, exposing employees to biological hazards.  c) Fabrication Shop - On or about March 31, 2021, employees handled and transferred corrosive materials including, but not limited to Sodium Metasilicate and Skyko Oshpho Rust Treatment, and were not provided a suitable eyewash station in that it was not inspected and maintained, exposing employees to biological hazards.
