29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about April 1, 2021, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about April 1, 2021, at the above addressed jobsite, employees performing roofing operations on a roof ten (10') eight (2") inches above the ground were not protected from falls, thereby exposing the employees to fall hazards.

On or about April 1, 2021, at the above addressed jobsite, employees performing roofing operations were using the ladder-vader/ platform hoist, which is intended to carry roofing materials, to access the roof, thereby exposing the employees to the hazards associated with falls of ten (10') feet.
