OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to a struck by/crushing hazard while using a Maxon TUK-A-WAY Series liftgate to unload a Thermatron Environmental Chamber from a moving truck:  6635 Kirkville Road, north side of building, northwest corner, Shipping/Receiving area, on or about 3-31-21:  One employee attempted to hold a large unit from rolling, putting himself in a position behind and below the 2,040 pound unit, exposing the employee to a struck by/crushing hazard.   Among other methods, one feasible and acceptable method of abatement to correct the hazard is:  Employers shall comply with Maxon GPT Series Operation Manual, M-17-07, Rev. B, August 2019, Warnings-page 6, paragraph 4, stating:  "Stand to the side of platform while operating the liftgate".  Employers shall comply with Maxon GPT Series Operation Manual, M-17-07, Rev. B, August 2019, Operation, page 22, Warning, stating:  "A load should never extend past the edges of the platform".  Employers shall provide specialized training specific to large and heavy industrial equipment and training on the recognition of the hazards associated with loading, moving, and unloading industrial equipment.
