Location: a) 106 Gardner Avenue, Brooklyn, NY 11237 / driveway a) On or about 04/02/2021, employees were using a JLG aerial lift to perform rigging operations on materials that were to be moved from a flatbed truck to the 2nd floor of the new building. Employees did not use a personal fall arrest system harness and lanyard while working from the JLG aerial lift.
