OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees where employees were exposed to crushing and struck-by hazards when using the AMMCO automotive lift model number B2900, serial number P970825854:  a) On or about April 1, 2021, and at times prior, the employer continued to operate and allow employees to operate an automotive lift without performing an inspection to identify a missing safety device and remove the item from service until replacement parts were installed.  The AMMCO two-pole automotive lift was designed to have an up-limit switch spanning the length of and under the steel cross-member; this lift was missing the up-limit switch. An opening in a stair rail system exceeded 19 inches at its least dimension:  a) On or about April 1, 2021, and at times prior, the employer exposed workers to falling from a stair system that ascended thirteen feet from the shop floor to the mezzanine level that had a twenty-five inch opening due to a missing mid rail.
