29 CFR 1910.28(b)(3)(i): Protection from fall hazards- Holes. The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: a) On or before April 2, 2021 the employer did not ensure that employees performing grocery picking activities on the level of the grocery picking area were protected from floor holes in that there were floor openings at the pass-throughs between the racks measuring approximately 18 inches by 3.5 inches.
