The employer required employees to wear respirators while providing care to suspected and confirmed positive COVID-19 residents and while performing anterior nasal COVID-19 tests on residents suspected of having COVID-19.

The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents and while performing anterior nasal COVID-19 tests on residents suspected of having COVID-19.

On or about January 8, 2021, an employee was hospitalized as a result of a confirmed case of COVID-19 and died on January 27, 2021. The employer did not record the work-related fatality on the OSHA 300 log or its equivalent for the calendar year 2021. b. On or about December 27, 2020 to December 31, 2020, the employer did not record the COVID-19 workplace illnesses on the OSHA Form 300 or equivalent for the calendar year 2020: There were eight incidents of COVID-19 infection that resulted in 10 to 14 days away from work; c. On or about January 1, 2021 to January 26, 2021, the employer did not record the COVID-19 workplace illnesses on the OSHA Form 300 or equivalent for the calendar year 2021: There were seventeen incidents of COVID-19 infection that resulted in 10 to 14 days away from work. Abatement Certification and Documentation is required
