29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: At the jobsite located at 8823 SW 8th St, Lot 62, Blue Springs, Missouri: An employee was exposed to fall hazards. An employee was lifted on the forks of a Genie GTH844 to the second story of the building approximately 9 feet above the ground. A safety platform/cage securely fastened to the lifting carriage and/or forks was not used.
