29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) On or about April 5, 2021, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded framed scaffold, sixteen (16) feet six (6) inches above the ground, was exposed to fall hazards due to the lack of a complete guardrail system including, but not limited to, a top rail at the cross brace and an end rail at the end of the outrigger platform.
