The employees are permitted to operate a piece of equipment with exposed rotating parts. On and about 4/05/2021 employees were exposed to rotating parts where the doors of the CNC-OKUMA LB15 were opened while the machine was in operation exposing the employees to amputation hazards.

On and about 4/05/2021 employees used compressed air for cleaning purposes. The pressure was measured at approximately 90 p.s.i. exposing the employees to hazardous conditions.
