29 CFR 1926.304(i)(1):Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut. On or about 4/06/2021 employees at the Forest Hills framing project were exposed to amputation hazards from an unguarded rotating table saw blade. Employees were operating a DeWalt table saw with no blade guard assembly.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. On or about 4/6/2021 employees at the Forest Hills framing project were not provided a complete guardrail system at the basement opening. The basement opening guardrail system was provided with a toprail but no midrail.
