29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a- 15 Somerset Drive, Suffern NY 10901: (Roof) An employee was engaged in gutter installation activities and exposed to fall hazards of approximately 18ft to the concrete below as an aluminum extension ladder used to access the roof whose ladder side rails did not extend 3ft above the upper landing surface. Condition noted on or about 04/06/2021
