29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) On or about 10/25/2020 - 6238 Clubhouse Way, Trussville, AL, employees sanding walls on the second floor of a residential house under construction were not protected from falling approximately 9 feet, 4 inches to the floor below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about 10/25/2020 - 6238 Clubhouse Way, Trussville , Alabama, employees engaged in activities, such as, but not limited to, sanding walls had not been trained.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   (a) On or about 10/25/2020 - 6238 Clubhouse Way, Trussville,  AL, the in-patient hospitalization of an employee that occurred on 10/25/2020 was not reported to OSHA until 04/02/2021.
