29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) UHC of S.E. Ohio located at 2980 Morse Road COLUMBUS, OH 43231: On and preceding April 1, 2021, the employer did not ensure that employees performing demolition work on a surface elevated 9 feet above the lower level were protected from falls. Two maintenance technicians were demoing a mezzanine including the removal of one side of the wall which created a 9 foot 7 inch wide section of open sided floor 9 feet above the lower level. The open side of the floor was left unprotected with no other means of fall protection being utilized as the maintenance technicians continued demolition and removal of appliances from the mezzanine at the open side of the floor. The following condition exposed employees to falls exceeding 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) UHC of S.E. Ohio located at 2980 Morse Road COLUMBUS, OH 43231: On and preceding April 1, 2021, the employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling. Two maintenance technicians were assigned to various tasks including but not limited to the demolition of a mezzanine that was 9 feet above the lower level. The employees were not trained to recognize and minimize fall hazards in the tasks they were assigned to perform which exposed them to falls greater than 6 feet.
