29 CFR 1926.451(b)(1):  Each Platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On 4/6/2021, employees from Heartland Restoration Inc. were repairing a brick commercial building located at 320 Main Street, Troy, MO.  The employees were working from a scaffold platform that was not fully planked or decked.

29 CFR 1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.  Crossbraces were used as a means of access:  On 4/6/2021, employees from Heartland Restoration Inc. were repairing a brick commercial building located at 320 Main Street, Troy, MO.  Employees did not have sufficient scaffold access to the upper working levels of the scaffold system.  Employees were climbing on the crossbracing to access the lower and upper levels.

The employee(s) were working from a tube frame scaffold system approximately 11 feet above the lower level.  Employees were not protected from falling to the lower level.

An Employee was working from a flat roof of the building near the leading edge/parapet wall approximately 13 feet above the lower level.  The employee was not protected from falling to the lower level by the use of guardrail systems, safety net systems, or personal fall arrest systems.

Employees were not trained by a qualified person in hazard recognition for the tube fame scaffold being used.  The employees did not show competence in hazards associated with working at heights on a scaffold.
