29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  On or about April 6, 2021, a laborer employee on the ground level working below five employees replacing plywood sheathing on an apartment building, was not provided a protective helmet.  This exposed the laborer employee performing clean-up to a struck-by hazard to the head from scrap plywood being thrown off the roof and/or other tools and equipment that may inadvertently fall from the roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:  (a)  On or about April 6, 2021, employees engaged in re-roofing (replacing sheathing) on an existing apartment building with a 6/12 pitched roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10 1/2 feet.
