The fall hazard to the employee ranged from approximately 9 feet to 26 feet

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite: On or about April 8, 2021, an employee was using a ladder to access the roof of a residential home and the ladder only extended about 2 feet above the roof. The fall hazard to the employee was about 9 feet.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection. (a) Jobsite: On or about April 8, 2021, the employer was not using either a ground fault circuit interrupter nor an assured equipment grounding conductor program on the site. An employee was using an extension cord between the permanent wiring of the house and a Black & Decker 7 1/4 inch circular saw.

This extension cord was in use to help supply electrical power to a Black & Decker 7 1/4 inch hand held circular saw.
