29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation  shall not be performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:  At a workplace located at 1788 Wildwood Drive in Suamico, Wisconsin:  On or about April 8, 2021, the Hyster forklift (S/N F005A04657H) had an unapproved attachment on the forks of the powered industrial vehicle to move materials and equipment in the yard and shop. The employer did not have the manufacturer's prior written approval to use the attachment and the plate on the vehicle had not been changed accordingly.

29 CFR  1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  At a workplace located at 1788 Wildwood Drive in Suamico, Wisconsin:  On or about April 14, 2021, the employer had not ensured that the forklifts were inspected daily to ensure they were in safe operating condition prior to placing them in service.

Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed.   At a workplace located at 1788 Wildwood Drive in Suamico, Wisconsin:  On or about April 8, 2021, in the main shop along the east wall, the working space about the electrical panels was not kept to a minimum of three feet for immediate access. An abrasive blast cabinet and large workbench were stored directly in front of the panel, blocking access.
