Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly: At a workplace located at 1788 Wildwood Drive in Suamico, Wisconsin: On or about April 8, 2021, the Hyster forklift (S/N F005A04657H) had an unapproved attachment on the forks of the powered industrial vehicle to move materials and equipment in the yard and shop. The employer did not have the manufacturer's prior written approval to use the attachment and the plate on the vehicle had not been changed accordingly.
